Scuba Diving Buoyancy Compensators Recalled by Ocean Management Systems Due to Drowning Hazard

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.

Name of Product: Buoyancy Compensators used for Scuba Diving

Units: About 20,000

Manufacturer: Ocean Management Systems Inc., of Middletown, N.Y.

Hazard: The buoyancy compensator seal ring could crack, posing a drowning hazard to divers.

Incidents/Injuries: None reported.

Description: This recall involves buoyancy compensators with the following model numbers. Buoyancy compensators provide buoyancy control for scuba divers by allowing them to inflate or deflate the devices. The compensators were sold in black or red. “OMS” is printed on the front inside of the compensators. Item and serial numbers are printed on the warning label located in the non-inflation area of the buoyancy compensator. A list of serial numbers included in this recall is available from the firm.

Dive Computers Recalled by Mares Due to Drowning Hazard

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission and Health Canada, in cooperation with the firm named below, today announced a voluntary recall of the following products. Consumers should stop using products immediately unless otherwise instructed.

Name of Product: Mares Nemo Air Dive Computers

Units: About 600 in the United States, 140 in Canada and 15 in Puerto Rico

Distributor: Mares USA, of Boca Raton, Fla.

Hazard: An O-ring in the high pressure air connector can fail and leak air, causing a continuous but slow loss of breathing gas, which could require a diver to surface quickly, posing a drowning hazard to divers..

Incidents/Injuries: None reported.

Description: This recall involves the Mares Nemo Air Dive Computer, Nemo Air Dive Computer with Compass, Mares High Pressure Hose with Quick Connector for Nemo Air, and Quick Connector Assembly for Nemo Air. These computers have a digital screen which allows scuba divers to measure the time and depth of a dive and process other information to help divers determine safe dive times and ascent rates.

Sold by: Specialty dive shops nationwide from July 2008 through July 2009 for between $800 and $900 (U.S.) and between $880 and $990 (Canadian).

Manufactured in: Italy

Remedy: Consumers should immediately stop using the recalled dive computer and connectors, and return the products to their authorized Mares dive shop for a free replacement O-ring connector assembly. The O-rings in some units may already have been replaced, but this recall requires replacing the metal quick connector fitting at the end of the high pressure air hose that holds the O-ring. Replacement connector assemblies have a groove machined around the middle of the fitting, but recalled units do not. All consumers should take their Nemo Air dive computers to a Mares dive shop to confirm whether this connector fitting has been replaced.

Tabata USA Recalls Scuba Regulators Due to Drowning Hazard

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following products. Consumers should stop using recalled products immediately unless otherwise instructed.

Name of Product: TUSA RS-670 Regulators

Units: About 250

Importer: Tabata USA Inc. (TUSA), of Long Beach, Calif.

Hazard: The first stage balance chamber plug can loosen from the scuba regulator causing a high-pressure leak and creating unstable pressure. This poses a drowning hazard to divers.

Incidents/Injuries: None reported.

Description: This recall involves R-600 first stage scuba regulators with the following serial numbers: UR600022 through UR600029, UR600031 through UR600103, UR6000637 through UR6000676, UR600708 through UR600716, UR600737 through UR600776. The serial number and TUSA logo are printed on the regulators.

Sold by: Authorized TUSA distributors and diving and equipment stores nationwide from May 2009 through September 2009 for about $450.

Manufactured in: Japan

Remedy: Consumers should immediately stop using the scuba regulators and return the product to TUSA or an authorized dealer for a free inspection and replacement.

Scuba Regulators Recalled by Cressi Due to Risk of Drowning

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission and Health Canada, in cooperation with the firm named below, today announced a voluntary recall of the following products. Consumers should stop using recalled products immediately unless otherwise instructed.

Name of Product: Cressi Ellipse Black MC5 Regulator

Units: About 200

Distributor: Cressi-sub USA, of Westwood, N.J.

Hazard: Partial obstruction of the High Pressure (HP) port can produce an inaccurate reading on the pressure gauge, resulting in a slow descent of the needle in the pressure gauge. The inaccurate reading on the gauge poses a drowning hazard to divers.

Incidents/Injuries: The firm has received three reports of inaccurate pressure readings while the scuba regulators were in use. No injuries have been reported.

Description: This recall involves regulators used to control the flow of air while scuba diving, making it possible for divers to breathe under water. The recalled regulators have model name Ellipse Black MC5. "Cressi MC5" and "Cressi Black" are printed on the regulator.

Sold at: Authorized dive shops nationwide and on the Internet from March 2009 through August 2009 for between $280 and $309.

Manufactured in: Italy

Remedy: Consumers should immediately stop using the regulators and return them to a Cressi authorized dive shop for a free repair. Consumers can also return the product to Cressi at One Charles Street, Westwood, N.J. 07675.

Diving Equipment Recalled by Dive Rite Due to Drowning Hazard

Name of Product: Dive Rite Wings

Units: About 16,000

Manufacturer: Lamartek Inc., dba Dive Rite of, Lake City, Fla.

Hazard: The over pressure valve springs in the diving equipment could rust and fail allowing the buoyancy compensator devices to leak, posing a drowning hazard to divers.

Incidents/Injuries: None reported.

Description: The Dive Rite Wings affected in this recall include the following models: Travel, Venture, Rec, Trek, Classic, Nomad and Super Wings. The wings were sold in red, blue or black. Springs were used on wings that have an opaque white or blue tinted bladder and welded in flanges. Wings with a black bladder are not affected. Only the Dive Rite wings that have a serial number range from 42000 through 72000 are affected by this recall. The serial numbers can be found on a white tag attached along a seam of the wing.

Sold at: Diving equipment retailers and distributors nationwide from June 2006 through October 2008 for about $359.

Manufactured in: United States

Remedy: Consumers should immediately stop using recalled diving equipment and return it to an authorized distributor or dealer for a free replacement of the overpressure valve spring.

Description: This recall involves Halcyon diving equipment including the Halcyon Explorer, Eclipse, CCR35, Evolve and Pioneer Buoyancy Compensator Devices (BCDs) and Halcyon Surface Marker Buoys (SMBs), Lift Bags, Diver Alert Markers (DAMs) Surf Shuttle and Diver Lift Raft Inflatable Devices. "Halcyon" is printed on the diving equipment.

Sold at: Diving equipment retailers and distributors from January 2006 through December 2008 for between $350 and $450 for the buoyancy compensator devices (BCDs) and between $50 and $275 for the inflatable devices.

Diving Wings With Defective Inner Bladders Recalled by Poseidon Due to Drowning Hazard

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.

Name of Product: Besea W50 Diving Wings with Poseidon Inner Bladders

Units: About 300

Distributor: Poseidon West, of West Lake Village, Calif., Poseidon Central, of Conroe, Texas, and American Divers Division, of Alexandria Bay, N.Y.

Hazard: The inner bladder located inside the diving wing can break, causing the wing to fail to operate as a floating/buoyancy device. This poses a drowning hazard to divers.

Incidents/Injuries: Poseidon has received 15 reports of inner bladders breaking. No injuries have been reported.

Description: The recall involves the Poseidon inner bladders with batch number 5445 sold with the Poseidon Besea W50 wings. The inner bladder is located inside the outer cover of the wing. The batch number and "Poseidon" are molded on a tab located between the "legs" of the inner bladder.

Sold at: Poseidon dealers nationwide from September 2007 through June 2008 for about $366.

Manufactured in: Sweden

Remedy: Consumers who have wings that contain the recalled inner bladders should stop using the wing immediately and contact Poseidon for a free replacement.

Aqua Lung Recalls Scuba Regulators and Adapters Due to Drowning Hazard

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.

Name of Product: Titan DIN 1st Stage Scuba Regulators and Titan/Conshelf DIN Scuba Adaptors

Units: About 6,000

Manufacturer: Aqua Lung USA, of Vista, Calif.

Hazard: Over-tightening of the DIN retainer by a technician during installation can result in the retainer breaking under pressure, a rapid escape of air from the scuba cylinder, and the regulator detaching from the scuba cylinder. This poses a drowning hazard to divers.

Incidents/Injuries: The firm has received one report of a DIN retainer breaking under pressure. No injuries have been reported.

Description: The recalled regulators have a brass DIN retainer manufactured prior to June 2006. The recalled Aqua Lung Titan Din 1st Stage regulators have serial numbers lower than 6062501 stamped on the side of the regulator’s body. Recalled Titan/Conshelf DIN adaptors are marked “300 BAR MAX” on the side of the part. DIN adaptors marked “300 BAR MAXI” are not included in this recall.

Sold at: Authorized Aqua Lung dealers nationwide from January 1997 through September 2008 for about $300 to $450 for the scuba regulators or $70 for DIN adaptors sold separately.

Manufactured in: France

Remedy: Consumers should immediately stop using the recalled diving regulators and DIN adaptors and return them to any authorized Aqua Lung dealer for a free replacement DIN retainer.

16 June 2008 Manufacturing error-WARNING!

WARNING

There seems to be a manufacturing error on the inner bladder that is mounted in our W50 wings, that causes the seem that holds the two inner bladder halves together, to break, making the wing unfit as a floating device.

It seems to be inner bladders marked with batch number 5445 (see picture below) that is effected by the manufacturing error.



These inner bladders have been mounted in W50 wings sold during the period: 2007-09-16 - 2008-06-10.

To check if you have an inner bladder effected by this manufacturing error, do the following:  
     
  • Between the two ´legs´ of the W50 wing, there is a zipper. Open this zipper.
  •  
  • You should now see the inner bladder. Between the two ´ legs´of the inner bladder, there is a tab with the article number, batch number and manufacturing year.
  •  
  • If the batch number on this tab reads 5445, the inner bladder might be defect.
  •  
  • If there is an other batch number, then the bladder is not effected by this information.
If you have an inner bladder from batch no 5445, we recommend that you DO NOT dive with the wing, until the inner bladder has been replaced!

Poseidon Diving Systems AB will replace all W50 inner bladders from manufacturing batch no 5445.

We ask you, who has an inner bladder from manufacturing batch 5445, to contact the dive store where you bought your W50 wing as soon as possible, so that your W50 bladder can be replaced as soon as possible.

Unfortunately, the manufacturing error was not detectable at the quality control that each inner bladder is put through prior to delivery.

Hazard:  Certain Dry suits that are equipped with inflation valves will also include the low pressure inflator hose.  There is a possibility that the low pressure hose quick connect will snap off of the hose. Shipment of hoses in question began on Sept. 1st, 2007.

SeaLife Announces Important Technical Update 24 Jan 2008

Name of Product:   SL961 Digital Pro Flash, SL168 - DC600 Elite (shipped from SeaLife commencing December 10, 2007), SL169 - DC600 Maxx Kit (shipped from SeaLife commencing December 10, 2007)

Manufacturer:  SeaLife

Hazard:  SeaLife recommends removing the spiral-sleeve protector or cables may break.

Incidents/Injuries:  Unknown.  It has been recognized by our Quality Control Department that the coiled sleeve which covers the cable inside the waterproof door can cause stress and possible damage to the cable connection. Repeated opening and closing of the door may
weaken and break this cable. If the cable does break , the flash will still fire but the flash power adjustment function would not work.

In order to eliminate this possible breakage, we have removed the wrap from production and are suggesting that the wrap be removed from any unsold inventory in your store. Attached you will find a technical update that will also be available on our website at, www.sealife-cameras.com , that gives simple instructions on how and what to remove.

Product shipped from SeaLife as of January 21, 2008, will have been updated and can be identified by a red dot sticker on the outside of the packaging. We will continue to identify these updated items with these dots until February 15,2008.

Hazard: An internal component that seals air between the high pressure first-stage and the intermediate pressure second-stage can fail. This can result in uncontrolled flow of air to the diver and pose a risk of serious injury or death.

Incidents/Injuries: ROMI has received two reports of units malfunctioning during diving and six reports of units malfunctioning while being tested by dealers prior to delivery to consumers. No injuries reported.

Hazard: The swivel, which is attached to a diving regulator, could separate while diving. This will result in a complete and sudden loss of the diver's air supply, causing the diver to engage in emergency ascent procedures. This poses a risk of decompression sickness due to rapid ascent, and air embolism or drowning if the diver panics or the emergency ascent procedure fails.

Incidents/Injuries: The firm has received one report where the swivel separated during a dive and caused the diver to ascend using the buddy breathing technique. No injury was reported.

Description: The recall involves the HO110 Swivels sold as an aftermarket regulator component. There are no markings such as a date or production code embossed on the unit. Contact Innovative Scuba Concepts for information on where the recalled units were sold.


Sold at: Specialty retail dive stores nationwide from January 2006 through March 2007 for about $40. They also could have been installed by a dive shop regulator technician.

Manufactured in: Taiwan

Remedy: Consumers should stop using regulators with the swivel attached and contact the dive store where purchased for a refund.

There maybe a potential problem with the yoke clamp screw on any yoke clamp type regulator with the serial number starting from 7010001 - 7053528. All Apeks dealers have been made aware of this issue.

Please return your Apeks regulator or just the yoke clamp screw to your nearest Apeks dealer for inspection. If you need assistance finding your nearest Apeks authorized dealer, please use the dealer locator found at www.aqualung.com. Do not dive with the regulator until it has been inspected.

Voluntary Recall of ATOM 2.0 Wrist Watch Dive Computers

Important safety issue involves ATOM 2.0 wrist watch dive computer.

February 12, 2007– San Leandro, CA – In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Oceanic Worldwide of San Leandro, California, is voluntarily recalling Oceanic ATOM 2.0 Wrist Watch Dive Computers. Oceanic has been notified by its supplier (Pelagic Pressure Systems) that they received isolated reports that two ATOM 2.0 Dive Computers locked into the Gas Switch-to screen and did not return to the Main Dive screen when the divers switched the units from Gas 1 to Gas d2 during dives when they were only using one Transmitter, and Pressure associated with Gas 2 was being provided by an Analog Gauge.

Affected are Oceanic ATOM 2.0 Wrist Watch Dive Computers having serial numbers 1 through 2079 that were sold between August 2006 and February 2007, and are being used for diving activities that include use of multiple gas mixes not associated with multiple transmitters.

Testing and evaluation has shown that this can occur if the control button used to switch Gases during a dive is depressed too long (4 seconds instead of the 2 seconds required to perform the operation) or it is pressed multiple times for 2 seconds when a Gas Switch operation is performed when there are multiple Gases (FO2 mixes) but not multiple Transmitters. Engineering programmers have modified the software of the ATOM 2.0 to prevent the Switch-to screen from locking on the display.

Oceanic asks that all owners of affected Oceanic ATOM 2.0 Wrist Watch Dive Computers return them to the Authorized Oceanic Dealer from which they were purchased for a software upgrade at no charge.

WARNING: DO NOT CONTINUE USE of the dive computers for multiple gas switching activities until the software is upgraded. Only Authorized Oceanic Dealers and their trained service technicians must perform this upgrade. Non-authorized personnel who attempt the upgrade procedure risk voiding the product's warranty and rendering the product dangerously unsafe for SCUBA use.

Depress the M (upper/left button) for 2 seconds to access the NORM Surface Main screen, the simultaneously depress the A (lower/left) and S (upper/right) buttons 4 times (2 seconds each time).

Scubapro® Recalls MK 20 Scuba Regulators for Service

The following product safety recall was voluntarily conducted by the firm in cooperation with the CPSC. Consumers should stop using the product immediately unless otherwise instructed.

Name of Product: Scubapro® MK 20 First Stage Regulators

Units: About 48,500 units.

Manufacturer: Scubapro® USA, of El Cajon, Calif.

Hazard: Over-tightening of the Yoke or Din retainer during annual servicing of these regulators could result in a stress crack and failure of the regulator. If this occurs during a dive, air supply could be interrupted, posing a drowning hazard.

Incidents/Injuries: No injuries or underwater incidents have been reported. Scubapro® USA has received eight reports of units that failed after being serviced.

Description: The recall involves first stage regulators that are marked with the Scubapro brand and identified as the MK20.

Sold at: Authorized Scubapro® dealers nationwide from January 1996 through December 2001 for about $260.

Manufactured in: Italy and United States.

Remedy: Scubapro® USA has been providing a service upgrade kit via the authorized dealer network since May 2005 which contains a Yoke or Din retainer that prohibits over-tightening during annual service. Consumers can easily identify if the service upgrade has been completed by counting the vents on the plastic saddle located on the regulator. If the service upgrade has not been completed, the consumer should bring the regulator to any authorized Scubapro® dealer to have the service upgrade completed free of charge.

Suunto Dive Computers Recalled Due to Decompression Hazard

Name of Product: Suunto D9 and D6 Model Wristop Dive Computers

Units: About 3,900

Manufacturer: Suunto Oy, of Finland

Hazard: These dive computers could incorrectly track dive time, which could cause incorrect calculation of decompression requirements. This could lead to decompression sickness.

Incidents/Injuries: None reported

Description: The D9 and D6 diving instruments are advanced, gas-switching, multi-mode, decompression dive wristop computers. The products included are: the D9 model with serial numbers 62102582 and below; and the D6 model with serial numbers 62103693 and below. The serial number is located on the side of the product. The model number is located on the back of the product.

Sold at: Diving specialty shops nationwide, as well as various Web sites, from September 2004 through June 2006 for about $1,275 for the D9 (without wireless transmitter) and $900 for the D6.

Manufactured In: Finland

Remedy: Consumers should only use these dive computers with backup instrumentation. Consumers should bring the recalled units to the nearest authorized Suunto dealer for a software update to correct the problem. A free battery replacement and pressure testing will be provided as part of the free software update service. All updated products will be marked with an indelible white dot on the back of the unit, or with an engraved “U” near the serial number.

Oceanic Recalls Digital Dive Computer Due to Decompression Hazard

Name of Product: Versa Pro 2A Digital Dive Computer

Units: About 6,100

Manufacturer: Oceanic, of San Leandro, Calif.

Hazard: When using the dive computer set for “User Selected Digital Gauge Mode,” the “Elapsed Dive Time” displayed can exceed the actual elapsed time under water. This can cause divers to ascend before fulfilling a decompression obligation, resulting in decompression sickness.

Incidents/Injuries: Oceanic has received one report of the dive computers malfunctioning. No injuries have been reported.

Description and Models: The recall involves Oceanic-brand Versa Pro 2A dive computers with serial numbers 12000 through 18176 which can be accessed and viewed on the computer’s display. The serial number and “r2A” are printed on the side of the unit. This recall does not include any other Oceanic brand dive computer, or Versa Pro 2A while operated in normal decompression dive computer mode.

Sold at: Authorized Oceanic dealers sold Versa Pro 2A digital dive computers nationwide from March 2003 through April 2006 for between $430 and $640.

Manufactured In: United States

Remedy: Consumers should stop using the recalled digital dive computers and take them to any Authorized Oceanic Dealer to get a free replacement Versa Pro 2B.

IMPORTANT SAFETY NOTICE REGARDING D9 AND D6 DIVING INSTRUMENTS 

June 9, 2006 ­ Suunto Oy today announced an important safety notice regarding its D9 and D6 diving instruments. This global decision involves D9 and D6 instruments that have been delivered to distributors and retailers, as well as product that has been sold to consumers.    

Quality and safety are paramount concerns for Suunto Oy. Our testing program for the D9 and D6 product by Suunto employees and independent divers all over the world has been extensive. Despite this, Suunto Oy has identified a software bug in the D9 and D6 instruments. The software bug may cause the D9 and D6 to incorrectly track dive time on rare occasions, potentially causing a risk to the diver.

The products affected are: 
D9 serial numbers 62102582 and below
D6 serial numbers 62103693 and below

The software bug may activate when the D9 and D6 automatically recalibrate to keep the correct time, or the user manually updates the seconds in set mode. Suunto approximates that there is 0.6 % (a six-tenths of one percent) chance that the software bug may appear during a dive. When the dive time is incorrect, every fourth second is not registered in the dive time and surface interval time, i.e., dive time goes 25% slower than actual time and tissue loadings are recorded at 25% less than actual loadings. As a trained diver you understand that this could affect your decompression requirements and that the length of a dive is one of the key factors in determining those requirements.

Even though the D9 and D6 will function correctly approximately 99.4% of the time during a dive, Suunto wants to eliminate the very small possibility of recording the dive time incorrectly. Therefore, Suunto is requesting that all D9 and D6 shall be returned for a software update that eliminates this issue. To date, there have been no known injuries related to this issue.

Name of Product: Scubapro® X650 Second Stage Regulator

Units: About 670

Manufacturer: Scubapro® with global headquarters located in El Cajon, Calif.

Hazard: A manufacturing error could cause the main housing of some regulators to change shape over time, which could cause the cover and diaphragm to become dislodged. If this occurs, air flow will be interrupted and the regulators will no longer function, posing a drowning hazard to users.

Incidents/Injuries: None reported.

Description and Models: The recall involves some X650 second stage regulators. All X650 second stage regulators are clearly marked with a blue capital “X” next to the numbers “650” in white. Affected regulators have a visible bump located on the right side of the unit between the letter “C” and where the mouthpiece attaches to the regulator body.

Oceanic Worldwide Announces Voluntary Recall of Versa Pro revision 2A Dive Computers
 
May 3, 2006 – San Leandro, CA – In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Oceanic Worldwide of San Leandro, California, is voluntarily recalling Oceanic Versa Pro revision 2A Digital Dive Computers. While operating in the User Selected Digital Gauge Mode, displayed Elapsed Dive Time can be in excess of actual elapsed time.
 
Oceanic has received a report of two Versa Pro revision 2A units that experienced the offset time displayed while operating in User Selected Digital Gauge Mode. No injuries have been reported.
 
Oceanic Versa Pro 2A Dive Computers subject to the recall are only the revision 2A units which have serial numbers 12000 through 18176 which is located on the side of the module and can be viewed if after the front button is depressed to activate the unit, it is held depressed until the Diagnostic Mode screen completes its countdown and the Serial Number screen appears displaying the module’s SN and software firmware revision (r2A). The unit will shut Off when released.
 
Authorized Oceanic dealers sold Versa Pro 2A modules, that were manufactured between March 21, 2003, and February 7, 2005, in wrist and console mount configuration during March 2003 through April 2006 for between $429 to $639, depending on the configuration with other instruments.
 
Consumers should immediately discontinue further use of Versa Pro 2A DiveComputers for User Selected Digital Gauge Mode operation (as a Depth Gauge/Timer) and take it to any Authorized Oceanic Dealer, or return it directly to the factory for an Upgrade to Versa Pro revision 2B.

Several 900 inflators have broken while in use. This creates the serious potential for letting the air out of the diver’s tank and potentially serious injury or death.

The recall 900 inflators are from two (2) production runs with date stamps “00/0” and “05/1”. Date stamp details are on next page.
 
The mouth piece is positioned (90 degrees) from the body of the inflator.

The mouth piece is located on the opposite side of the body from the Low Pressure hose connection.

Product: Aqua Scout Water Scooters

Hazard: Hydrogen gas can build up in the battery compartment and cause the battery cover and the battery package to forcefully expel from the product, posing a risk of injury to the user or bystanders.

Incidents/Injuries: Aqua Scout has received five reports of the battery cover and/or the battery package being expelled from the water scooter, including two reports of minor injuries such as lacerations and bruising.

Description: The Aqua Scout Water Scooter is a hand-held, battery-powered product used to propel swimmers through the water. The product has a yellow or red plastic enclosure, a black plastic handle and propeller. There is no model numbers or brand name written on the scooter.

Sold at: Online at the Aqua Scout Web site and through eBay from May 2005 through July 2005 for about $120.

Manufactured in: China

Remedy: Consumers should stop using the recalled water scooter immediately and return it to Aqua Scout for a free replacement.

Aftermarket Swivel for AGA/Interspiro Divator MKII Full Face Mask

M&J Engineering Voluntary Recall of SW-3000 2nd Stage Swivel with Stamped date of 01/04 to 02/05

M&J Engineering voluntarily conducted the following product safety recall. Consumers should stop using the product immediately and contact M&J Engineering.

Name of Product: Swivel SW-3000

Products Affected: All SW-3000 Swivels that are stamped with the manufacturing date of 01-04 thru 02-05

Manufacturer: M&J Engineering

Hazard: The Swivel has the potential to become unscrewed while in use and at depth on the Interspiro Mask. This recall is for the AGA Nut and Washer only. It has come to our attention that there is a defect in the washer. The washer is too thick for the spec range and has the potential to lock against the nut while under pressure. While the insert is locked against the washer it has the potential to act as a ratcheting affect and will back the swivel nut off of the regulator.

Incidents/Injuries: There have been a few incidents that have been recently brought to our attention that the swivel in the above described dates have unscrewed from the face mask while at depth. There have been no reports of injury.

Description: The recall involves all SW-3000 2nd stage 360-degree swivels with the stamped manufacturing date of 01/04 thru 02/05. The Stamp is marked on the Swivel body underneath the Patent number.

How to Identify the Problem:

To identify which swivels are potential problems please conduct the following steps. Screw the swivel onto the mask 2nd stage regulator. Hand tighten the swivel until it is snug. The Swivel should be able to move in and out, pushing the ball of the swivel in towards 2nd stage regulator. It should move approximately 20-30 thousandths of an inch, (.020 to .030) or the approx thickness of 2-3 business cards. The Swivel should also rotate inside the body of the regulator smoothly with very little resistance. This is natural resistance from the o-ring inside the regulator body. If there is movement in and out and the swivel is free moving inside the body of the regulator proceed to the next step.

If there is no in and out movement or the Swivel locks tight when screwed down, STOP use of the swivel IMMEDIATELY and refer to Remedy below.

Next connect the system to an air tank. Apply standard pressure to the system as if you would normally do during your pre-dive check. Hold the hose and the mask. Move the mask around in a circular motion as to imitate the normal movement of your head while you are underwater. After you have done this at least five (5) times stop and check the swivel nut to see if it as backed of any. You will be able to thread the swivel down some. While the system is charged with air you will still be able to thread the swivel down if any movement has occurred. If the swivel has moved at all STOP use of the swivel IMMEDIATELY and refer to Remedy below. This is the swivel must have the washer replaced.

Remedy: The washer will be replaced by manufacturer free of cost. In some instances it maybe necessary to replace the AGA Nut. If this is necessary the manufacturer will do so free of charge. Please send all SW-3000 AGA Swivels within date range stated above to M&J Engineering for replacement. All products must have a RM# Prior to acceptance. Regulator Assembly
We have discovered an inconsistency with some of our regulator assemblies P/N 505-069 that were manufactured between January 1, 2005 to March 1, 2005. The tube that holds the inlet valve and lever arm on some of these units in this batch are misaligned and consequently the lever arm sits offset. This offset if large enough will not allow the lever to travel smoothly and may increase the inhalation effort. Regulators that have this configuration should be taken out of service and not used until they have been replaced.  Figures on  http://www.kirbymorgan.com/


In Fig.1 note the parallel line drawn that represents the horizon. The roller lever’s correct position is parallel to that.


In Fig.2 note the parallel line drawn that represents the horizon. The roller lever is out of position at an angle that exceeds three degrees.


Note that some regulators may have roller levers that appear to be out of alignment by one or two degrees. This is within acceptable tolerances and performance and these regulators do not need to be returned for replacement.

If you discover that you have any of these products with the offset lever arm or inconsistent tube spacing, please send them back to KMDSI and we will replace them at no charge to you as soon as possible. If you have helmets and/or bandmasks with this incorrectly assembled regulator and you have an Authorized KMDSI Technician available, please remove the regulator and return it for replacement. We have corrected this situation and do not anticipate any further inconsistencies.

Hazard: A slow leak from the shoulder exhaust caused by expansion of an
internal cable housing could result in slow deflation. This could impact
the diver's ability to control buoyancy.
 
Incidents/Injuries: None reported.
 
Description:
Buoyancy Compensator devices provide buoyancy control for
scuba divers by allowing them to inflate or deflate the devices.
The recalled buoyancy compensators have model numbers J107, J113, J115, and J119. The numbers appear on the top right-hand corner of the unit (on
the shoulder). "CRESSI" is written on the buoyancy control devices.
 
Sold at:
Authorized dive shops nationwide and at various Web sites from January 2003 through September 2004 for between $400 and $700.
 
Manufactured in: Genoa, Italy.
 
Remedy: Customers can return the product to a Cressi-sub-authorized dive
shop to have the product retro-fitted with a free replacement internal
cable housing. Buoyancy Compensator devices with a black retaining ring
around the yellow inflate button on the inflation mechanism, and an
adjoining gray cover assembly (bypass case), which screws onto the
inflator, are subject to this recall and must be retro-fitted. Devices
which have yellow retaining rings and black cover assemblies have been
retro-fitted.

Hazard: The dive computers were improperly calibrated, resulting in incorrect calculations. Diving with an improperly calibrated dive computer can provide inaccurate “No Decompression (no stop) Time,” “Decompression Time,” “Desaturation Time” and “No Fly Time.” This could result in divers suffering serious injuries, including decompression sickness.

Incidents/Injuries: Head USA received one report of a computer malfunction. No injuries were reported.

Description: The recall includes the Dacor Darwin Air dive computer with serial number 3265 through 3415, and the Dacor Darwin Console dive computer with serial numbers 8503 through 8592. The serial number will appear on the dive computer screen after removing and reinserting the batteries. The Darwin Air is a completely integrated dive computer and pressure gauge with “Dacor Darwin Air” written on the front. It is encased in gray on black heavy-duty plastic. The Darwin Console is a dive computer with a compact submersible pressure gauge bearing the word “Dacor” on the front. It is encased in black heavy-duty plastic.

Sold at: Dive shops nationwide from March 2004 through December 2004 for about $700 for the Darwin Air and $400 for the Darwin Console.

Manufactured in: Italy by HTM Sports S.p.A.

Remedy: Consumers should immediately stop using these recalled diving computers, and return the computers to an authorized Dacor dealer for a free repair.

Suunto have issued a safety notice about the new D9 computer.

SAFETY NOTICE REGARDING SUUNTO D9 DIVE COMPUTERS

Suunto production tests have found a software fault in the first series of Suunto D9 computers (products with serial numbers 4xxxxxxxx - 450000699, software version 1.x.x - 1.2.4). Suunto advice is that the above numbered D9 computers are not dived with before a software update has been made.

Although highly unlikely, the fault may under certain circumstances cause the depth and time display to update slower than normal. Any potential problems are avoided by upgrading the D9 software to version 1.2.8. There have been no reports of users experiencing this fault.

In total less than 40 D9 have been supplied to UK dealers, all with the software fault. Suunto UK knows all the end users - mostly dive professionals -and every one has already been contacted to arrange the software upgrade. This upgrade will be done at Suunto UK office.

Any other UK owner of a D9 that needs the software upgrade is advised to also send it directly to Suunto in UK. The upgrade service will be operational from 10 January. Turnaround will be quick, with the upgrade done by reprogramming the D9's flash memory.

The first UK shipment of D9 available for public sale, with the updated software, is expected to be available in dealers before end January.

If you have recently purchased a Dacor Darwin Air or Darwin Console Dive Computer, please read the following safety notice regarding the recall of certain computers.

HTM Sport S.p.A., Manufacturer of Dacor Dive Computers, has discovered that during production, a small number of Dacor Darwin Dive Computers were improperly calibrated, resulting in erroneous calculations displayed regarding "No Deco Time", "Deco Time", "Air Time", "Desat Time" and "No Fly" times.

The specific products that may have this defect are in the following groups:

Darwin Air Dive Computers    Serial Numbers between: 3265 and 3415

Darwin Console Computers    Serial Numbers between: 8503 and 8592

To locate the serial number of your Darwin Air or Darwin Console:

1. Find the watertight battery compartment cover located on the top of Darwin Air or the left side on the Darwin console.
2. Unscrew the watertight cover by turning 90° counter clockwise.
3. Remove the batteries and then put them back in, after a few seconds the serial number of the instrument will appear on the screen.

If you own one of the Darwin Computers with a serial number listed above,
DO NOT USE IT!

Diving with an improperly calibrated dive computer will provide inaccurate No Decompression (no stop) Time, Decompression Time, Desaturation Time and No Fly Time, and could result in serious injury, including Decompression Sickness due to Omitted or Insufficient Decompression!

Immediately take your dive computer to your Authorized Dacor Dealer for repair. You should expect to receive it back from the dealer within 4-6 weeks.

May 25, 2004 Halcyon Stainless Steel Power Inflator Recall

Halcyon is recalling all stainless steel power inflators. 
The only units affected can be easily identified by their stainless buttons
and started shipping on all Halcyon BCs after Oct 21, 2003. This recall is
in response to problems experienced by some Halcyon customers.
Identification of the cause was unusually difficult due to the odd
dispersion of these problem units. Furthermore most customers report that
their units are performing flawlessly. However, there are pockets of
unacceptable failures in which customers or dive shops seem to have a
grouping of unreliable inflators. Given the nature of this failure, there
is no way to ensure that a unit performing properly will not develop
problems over time.

Typically, the problem expresses itself as a slow leak into the BC bladder.
The cause of the leak relates to small imperfections within the machining
of the stainless air barrel of some inflators. Not all of the inflators
exhibit the machining flaws. Thus, a majority of the units perform
flawlessly but groupings of product might display the imperfections. We
believe there to be no responsible alternative but to institute a recall of
every stainless steel inflator sold by Halcyon. In order to ensure this
recall occurs in the most responsible manner possible, we have contacted
the U.S. Products Safety Commission and notified them of the return. They
are monitoring the recall process and the return of the units.

Halcyon is deeply apologetic for any inconvenience this recall might cause.
We are dedicated to making the world's best diving products and to ensuring
diver safety at all costs; this solution seems the only reasonable course
of action. Please do not use your BC with the Halcyon stainless steel power
inflator until we have had the chance to replace the suspect unit. This
recall only applies to Halcyon inflators shipped after Oct 21, 2003. These
units have stainless steel buttons and are easily identifiable from
previous Halcyon inflators.

In order to reduce the inconvenience and to return your BC to operation as
quickly as possible, we are shipping new inflators to your local Halcyon
dealer. You may bring your inflator to your dealer, trade it for a new
unit, and have it fitted to your buoyancy compensator. Please contact your

April 8, 2004 Aqua Lung ABS Octopus Exhaust Grid Replacement

Aqua Lung has determined that it is possible, under certain circumstances, for one or both of the exhaust valves on the ABS Octopus to become inverted.  Inverted means that the edge of the exhaust valve can get lodged between two of the spokes of the seating surface.  This could happen during the after-dive rinsing process if the valves are hit with a stream of high pressure water from a garden hose or it the valves hit the water just right during an entry.  The result of having one or both valves inverted is the inability to completely seal out water.  This could lead to an aspiration of water, which in turn could lead to panic, injury or death. The valves will not invert during the course of a dive. To protect the valve from inverting, Aqua Lung has redesigned the exhaust grid that covers the exhaust valves (see picture on website)

Aqua Lung is requesting that all ABS Octopus regulators be brought into your local authorized Aqua Lung Dealer.  The dealer will change out the exhaust grid at no charge.  The replacement procedure is quick and in most cases can be done while you wait.  The procedure is non-invasive and will not alter the performance or the adjustments of the regulator in any way. We have been informed by our Research and Development department of the possibility of cracks appearing in the battery compartment door or/and in
the USB compartment door of the DX-3100 Aquapix amphibious camera that may lead to water leakage. Although we have only received a limited number of these reports, even the remotest possibility of this condition appearing has prompted Sea & Sea to take affirmative and positive action and therefore we would like to request that you return your DX-3100 Aquapix to the Sea & Sea dealer where it was purchased, or to an official Sea & Sea distributor for testing and any necessary upgrading if within these corresponding serial numbers.

3/16/2004 Powerline Airway Ribbed Hose Replacement

Aqua Lung has received reports that some of the ribbed hoses (Fig. 1) on the Powerline Airway have been tearing at the top near the dual exhaust valve. The problem has been traced to an error in the molding process from our supplier that occurred over a three-month period between 2001 and 2002. The best way to confirm whether or not you have one of the questionable hoses is to look at the serial number on the lower unit of the Powerline (fig. 2). If the serial number falls within the range of M48H to M52H (M48H, M49H...M52H), or M01I to M08I (M01I, M02I...M08I) and is the original hose, then it should get replaced. This affects both SeaQuest and Aqua Lung BCs utilizing the Powerline inflator.

If the ribbed hose were to tear near the top, the BC would not be able to hold air and a loss of buoyancy would occur. This could result in the need for the diver to ditch their weight pockets or rely on their buddy for assistance. In extreme cases it could result in result in panic, injury or death.

If you have a spiral hose that falls within the serial number range, please have the hose replaced by this authorized Aqua Lung retailer.

SHERWOOD SCUBA / GENESIS SCUBA
BUOYANCY COMPENSATOR POWER INFLATOR VALVE

This recall has been reported to the US Consumer Product Safety
Commission with whom we are work working and who will monitor its
effectiveness.

A condition has recently been identified that may exist in recently
manufactured Sherwood Scuba and Genesis Scuba Buoyancy Compensators (BCD)
that requires immediate attention.  The power inflator on certain model BCDs
could, in rare instances, provide continuous inflation and expose the diver
to potential injury if appropriate measures are not taken to operate the
exhaust valve to control ascent.
The condition is caused by a molding variation in a seal within inflator
valve, and although this variation is not present in every power inflator,
we are requiring that all BCDs manufactured within a limited time period be
updated with new power inflator valve assemblies.
If you believe your BCD may be included in the group of affected products,
discontinue use for diving immediately and contact your authorized Sherwood
or Genesis dealer for a free upgrade or contact our toll-free number for
assistance.

AFFECTED BUOYANCY COMPENSATORS:
Power Inflator Valves within the specified serial number range having either
a black or gray colored power inflator button.  The updated power inflator
valve assembly will have a blue colored button.
Buoyancy compensator power inflators with a blue colored power inflator
button are not affected by this notice.

Effective immediately, Light & Motion announces a Safety Recall on the Sunray Mod Light.

There have been a few incidences in which the light inadvertently turned on while unattended. It has been determined that if the light accidentally becomes partially flooded, it is possible for the light to turn on by itself. If you suspect the light has been flooded, it is important to unscrew the silver bezel and remove the light bulb from the socket.

Software in the dive computers may inaccurately calculate desaturation times, resulting in possible decompression sickness.

  UWATEC AG has received five reports of decompression sickness allegedly associated with the use of the 1995 dive computers.

  This recall involves Aladin Air X NitrOx dive computers manufactured in 1995. The manufacture date is located on the back of the dive computer, in the lower right hand corner. The date code reads the number of the month followed by a decimal point and the year, for example, "01.95." The dive computers are blue and black with the words "Aladin" and "NitrOx" printed across the front. Aladin Air X NitrOx dive computers with date codes other than "95" are not included in this recall.

  Scuba diving stores sold the dive computers nationwide from July 1995 through March 1996 for between $950 and $1,200.

  WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Oceanic USA, of San Leandro, Calif., is recalling about 24,700 Oceanic CDX first stage regulators used for scuba diving.  Extreme vibration can occur within these CDX regulators, which can cause an air leakage underwater. Divers could run out of air and drown.

  Oceanic has received six reports of noise or vibration accompanied by air leakage. No injuries have been reported. 

  Oceanic CDX first stage regulators subject to the recall have serial numbers 9200001 to 9205622, 9800013 to 9801711, 0200001 to 0213294, 0D0001 to 0D3046, or 9D0001 to 9D3273. The serial number is stamped into the side of the body of the regulator. The regulators have the name "OCEANIC" written on the rubber boot of the first stage. These CDX regulators were sold with the following second stage regulators: Alpha 7, Delta 3, Gamma 2, and Zeta. 

  Authorized Oceanic dealers sold these regulator sets worldwide from May 1999 through October 2002 for between $330 to $640, depending on the second stage.

  Consumers should stop using the recalled CDX regulators immediately and take them to any authorized Oceanic dealer to get a free repair. WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Johnson Outdoors
Inc., of Racine, Wis., is voluntarily recalling about 1,700 SCUBAPRO/UWATEC™ dive computer consoles. The U-Line™
submersible pressure gauge that is part of the dive consoles can malfunction and display inaccurate pressure readings. Divers
using the consoles could fail to decompress properly during a dive, causing decompression sickness. 

CPSC and Johnson Outdoors Inc. have received four reports of the pressure gauge malfunctioning. No injuries have been reported.

Only U-Line™ submersible pressure gauges contained in UWATEC™ brand Smart PRO™, Pro ULTRA™, and Sport PLUS™ dive computer consoles are included in the recall. The consoles are mostly gray in color and the brand name
"UWATEC™" is written on the front of each console. The pressure gauges are made in the U.S. and the computers are made
in Switzerland. All of the recalled consoles have a test code that appears on the back of the gauge as follows:

 Abysmal Diving Inc. of Lake Havasu City, Arizona has been notified that the above scuba cylinder valves manufactured and sold between January 2000 and July 9, 2002 may contain one or more materials that may not be compatible with 100% pure Oxygen (when introduced at 250psi and above) or Oxygen Enriched Air Nitrox mixtures greater than 21% oxygen content.

 You must stop using the valve and manifold combinations listed above immediately if being used with 100% pure high pressure oxygen (200 psi or more) or oxygen enriched air mixtures greater than 21% oxygen content or if the valve or manifold is exposed to high pressure oxygen (200 psi or greater) during a partial pressure mixing process.  You must immediately return the item (s) to Abysmal Diving Inc. where they will be serviced and returned to you at no charge.

 If you now use or intend to use your valve and scuba tank for AIR use, only, and if you have no intention of introducing 100% pure Oxygen (when introduced at 200psi and above) or Oxygen Enriched Air Nitrox mixtures greater than 21% oxygen content through it, then this Product Safety Notice does not apply to you.

 INSTRUCTIONS:

 If your valves are subject to this Product Safety Notice, please e-mail, call or write Abysmal Diving Inc. immediately at:
 2099 Acoma Boulevard West, Suite C, Lake Havasu City, Arizona, 86403 USA Phone: (928) 854-9470 Fax: (928) 854-9473, and request an SMA # (Service Merchandise Authorization #). Then pack your valves in an appropriate container and return them to Abysmal Diving Inc. When your valves arrives they will undergo a complete tear down, inspection, cleaning and will be fitted with any required oxygen compatible materials and lubricants. Your serviced valves will then be shipped back to you free of charge.

 As of July 20th, 2002 Abysmal Diving Inc. has received no reports of any valve failing or exhibiting any oxygen compatibility problems. However this Product Safety Notice has been issued and a free retrofit offer made, in an attempt to ensure our customers have all the components originally intended in these valves for oxygen/nitrox service. Abysmal Diving Inc. strives to produce the finest and safest diving equipment, and protect its customers from the unreasonable risk of injury.

 Again, if you currently introduce 100% pure oxygen or oxygen enriched air gas mixtures with an oxygen content greater than 21% you must immediately discontinue use of the above listed valves until such time as they have been returned to Abysmal Diving Inc. for a complete inspection and any required retrofitting and preparation for oxygen service.
 DO NOT CONTINUE TO USE THESE VALVES IN OXYGEN-RELATED SERVICE BECAUSE OF THE POSSIBILITY OF FIRE AND SERIOUS INJURY OR DEATH.

CPSC, Sherwood SCUBA LLC Announce Recall of Maximus Regulators

WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product SafetyCommission (CPSC), Sherwood SCUBA LLC, of Irvine, Calif., is recalling about 14,000 Maximus SCUBA regulators. The second stage orifices on these regulators can be cracked, bent or broken. This can result in a loss of breathing air underwater.

 Sherwood SCUBA has received seven reports of broken second stage
orifices on these regulators. Two of the regulators broke during dives
underwater. No injuries were reported.

 The Maximus regulators included in this recall have the following
model, serial numbers and connection types:

Model Number: SRB5600
Serial Number Range: K600001 through K611834
Description: Yoke Connection

Model Number:SRB5600D2
Serial Number Range: DK60000 through DK62000
Description: Din-Style Connection

Model Number: SRB5600CE
Serial Number Range: EK60001 through EK62000
Description: Yoke Connection

The serial numbers are laser marked on the top of the second stage case
next to the mouthpiece.

 Authorized Sherwood SCUBA distributors and dealers nationwide sold
these regulators from January 1998 through November 2000 for about $580.

 Consumers should stop using the recalled regulators immediately, and
contact their nearest authorized Sherwood retailer for a free repair.

April 30, 2002 Subject: Product Safety Recall -OMS end cap

  Product Safety Recall : Part number V 112 
  Attn: Dive stores and end users. This cap as shown in the Basic 
  Valves page is under recall. Please take this cap out of service 
  immediately as it can non-passively fail if over torqued.

  These caps will be replaced immediately at no charge by OMS upon 
  receipt of the previous model V112 with a new "rounded cap" 

  The new "rounded" cap is to be hand tightened, and has a 6mm Hex head 
  for measured torqueing purposes (110 inch lbs).

Jan 30, 2002 In cooperation with the U.S. Consumer Product Safety
Commission (CPSC), Custom Buoyancy Inc. (CBI), of Torrance, Calif., is voluntarily recalling about 7,700 overpressure valves (OPV) used with buoyancy control systems.  The overpressure valve can stick in the open position, posing a drowning hazard to divers.

The recalled overpressure valve is installed on buoyancy control
systems with the following brand names: Diving Unlimited International (DUI), International Divers, Ocean Management Systems, Rip Tide and Steam Machines. Brand names and logos are printed on the epaulettes and pockets.

November 24, 2001 SAFETY RECALL on DUI B.C.D.'s 
Washington, D.C.--In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Diving Unlimited International, Inc. (DUI) of San Diego, California, is recalling approximately 3,500 Buoyancy Control System Overpressure Valves.

Molding variations in one component of the overpressure valve (OPV) used in the DUI Buoyancy Control Systems (BCS) could, in rare instances, prevent the OPV from sealing properly and creating a potential drowning hazard. CPSC and DUI have received no reports of injuries due to this malfunction.
