Aqua Lung Recalls Scuba Regulators and Adapters Due to Drowning HazardWASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.Name of Product: Titan DIN 1st Stage Scuba Regulators and Titan/Conshelf DIN Scuba Adaptors Units: About 6,000 Manufacturer: Aqua Lung USA, of Vista, Calif. Hazard: Over-tightening of the DIN retainer by a technician during installation can result in the retainer breaking under pressure, a rapid escape of air from the scuba cylinder, and the regulator detaching from the scuba cylinder. This poses a drowning hazard to divers. Incidents/Injuries: The firm has received one report of a DIN retainer breaking under pressure. No injuries have been reported. Description: The recalled regulators have a brass DIN retainer manufactured prior to June 2006. The recalled Aqua Lung Titan Din 1st Stage regulators have serial numbers lower than 6062501 stamped on the side of the regulator’s body. Recalled Titan/Conshelf DIN adaptors are marked “300 BAR MAX” on the side of the part. DIN adaptors marked “300 BAR MAXI” are not included in this recall. Sold at: Authorized Aqua Lung dealers nationwide from January 1997 through September 2008 for about $300 to $450 for the scuba regulators or $70 for DIN adaptors sold separately. Manufactured in: France Remedy: Consumers should immediately stop using the recalled diving regulators and DIN adaptors and return them to any authorized Aqua Lung dealer for a free replacement DIN retainer. Manufacturing error-WARNING! By: Poseidon 2008-06-16 WARNING There seems to be a manufacturing error on the inner bladder that is mounted in our W50 wings, that causes the seem that holds the two inner bladder halves together, to break, making the wing unfit as a floating device. It seems to be inner bladders marked with batch number 5445 (see picture below) that is effected by the manufacturing error.
These inner bladders have been mounted in W50 wings sold during the period: 2007-09-16 - 2008-06-10. To check if you have an inner bladder effected by this manufacturing error, do the following:
Poseidon Diving Systems AB will replace all W50 inner bladders from manufacturing batch no 5445. We ask you, who has an inner bladder from manufacturing batch 5445, to contact the dive store where you bought your W50 wing as soon as possible, so that your W50 bladder can be replaced as soon as possible. Unfortunately, the manufacturing error was not detectable at the quality control that each inner bladder is put through prior to delivery. Whites Recalls LP Hose February 18th 2008 Name of Product: Quick Connect LP Inflate Hose 29", VLSITINF 32120 Manufacturer: Whites Hazard: Certain Dry suits that are equipped with inflation valves will also include the low pressure inflator hose. There is a possibility that the low pressure hose quick connect will snap off of the hose. Shipment of hoses in question began on Sept. 1st, 2007. Incidents/Injuries: None reported. Technical Update 24 Jan 2008 Name of Product: SL961 Digital Pro Flash, SL168 - DC600 Elite (shipped from SeaLife commencing December 10, 2007), SL169 - DC600 Maxx Kit (shipped from SeaLife commencing December 10, 2007) Manufacturer: SeaLife Hazard: SeaLife recommends removing the spiral-sleeve protector or cables may break. Incidents/Injuries: Unknown. It has been recognized by our Quality Control Department that the coiled sleeve which covers the cable inside the waterproof door can cause stress and possible damage to the cable connection. Repeated opening and closing of the door may weaken and break this cable. If the cable does break , the flash will still fire but the flash power adjustment function would not work. In order to eliminate this possible breakage, we have removed the wrap from production and are suggesting that the wrap be removed from any unsold inventory in your store. Attached you will find a technical update that will also be available on our website at, www.sealife-cameras.com , that gives simple instructions on how and what to remove. Product shipped from SeaLife as of January 21, 2008, will have been updated and can be identified by a red dot sticker on the outside of the packaging. Hazard: An internal component that seals air between the high pressure first-stage and the intermediate pressure second-stage can fail. This can result in uncontrolled flow of air to the diver and pose a risk of serious injury or death. Incidents/Injuries: ROMI has received two reports of units malfunctioning during diving and six reports of units malfunctioning while being tested by dealers prior to delivery to consumers. Voluntary Recall of ATOM 2.0 Wrist Watch Dive Computers Important safety issue involves ATOM 2.0 wrist watch dive computer. February 12, 2007– San Leandro, CA – In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Oceanic Worldwide of San Leandro, California, is voluntarily recalling Oceanic ATOM 2.0 Wrist Watch Dive Computers. Oceanic has been notified by its supplier (Pelagic Pressure Systems) that they received isolated reports that two ATOM 2.0 Dive Computers locked into the Gas Switch-to screen and did not return to the Main Dive screen when the divers switched the units from Gas 1 to Gas d2 during dives when they were only using one Transmitter, and Pressure associated with Gas 2 was being provided by an Analog Gauge. Affected are Oceanic ATOM 2.0 Wrist Watch Dive Computers having serial numbers 1 through 2079 that were sold between August 2006 and February 2007, and are being used for diving activities that include use of multiple gas mixes not associated with multiple transmitters. Testing and evaluation has shown that this
can occur if the control button used to switch Gases during a dive is
depressed too long (4 seconds instead of the 2 seconds required to
perform the operation) or it is pressed multiple times for 2 seconds
when a Gas Switch operation is performed when there are multiple Gases
(FO2 mixes) but not multiple Transmitters. Engineering programmers have
modified the software of the ATOM 2.0 to prevent the Switch-to screen
from locking on the display. WARNING: DO NOT CONTINUE USE of the
dive computers for multiple gas switching activities until the software
is upgraded. Only Authorized Oceanic Dealers and their trained service
technicians must perform this upgrade. Non-authorized personnel who
attempt the upgrade procedure risk voiding the product's warranty and

Consumers should stop using the product immediately unless otherwise instructed. Name of Product: ScubaproŽ MK 20 First Stage Regulators Units: About 48,500 units. Manufacturer: ScubaproŽ USA, of El Cajon, Calif. Hazard: Over-tightening of the Yoke or Din retainer during annual servicing of these regulators could result in a stress crack and failure of the regulator. If this occurs during a dive, air supply could be interrupted, posing a drowning hazard. Incidents/Injuries: No injuries or underwater incidents have been reported. ScubaproŽ USA has received eight reports of units that failed after being serviced. Description: The recall involves first stage regulators that are marked with the Scubapro brand and identified as the MK20. Sold at: Authorized ScubaproŽ dealers nationwide from January 1996 through December 2001 for about $260. Manufactured in: Italy and United States. Remedy: ScubaproŽ USA has been providing a service upgrade kit via the authorized dealer network since May 2005 which contains a Yoke or Din retainer that prohibits over-tightening during annual service. Consumers can easily identify if the service upgrade has been completed by counting the vents on the plastic saddle located on the regulator. If the service upgrade has not been completed, the consumer should bring the regulator to any authorized ScubaproŽ dealer to have the service upgrade completed free of charge. Name of Product: Suunto D9 and D6 Model Wristop Dive Computers Units: About 3,900 Manufacturer: Suunto Oy, of Finland Hazard: These dive computers could incorrectly track dive time, which could cause incorrect calculation of decompression requirements. This could lead to decompression sickness. Incidents/Injuries: None reported Description: The D9 and D6 diving instruments are advanced, gas-switching, multi-mode, decompression dive wristop computers. The products included are: the D9 model with serial numbers 62102582 and below; and the D6 model with serial numbers 62103693 and below. The serial number is located on the side of the product. The model number is located on the back of the product. Sold at: Diving specialty shops nationwide, as well as various Web sites, from September 2004 through June 2006 for about $1,275 for the D9 (without wireless transmitter) and $900 for the D6. Manufactured In: Finland Remedy: Consumers should only use these dive computers with backup instrumentation. Consumers should bring the recalled units to the nearest authorized Suunto dealer for a software update to correct the problem. A free battery replacement and pressure testing will be provided as part of the free software update service. Name of Product: Versa Pro 2A Digital Dive Computer Units: About 6,100 Manufacturer: Oceanic, of San Leandro, Calif. Hazard: When using the dive computer set for “User Selected Digital Gauge Mode,” the “Elapsed Dive Time” displayed can exceed the actual elapsed time under water. This can cause divers to ascend before fulfilling a decompression obligation, resulting in decompression sickness. Incidents/Injuries: Oceanic has received one report of the dive computers malfunctioning. No injuries have been reported. Description and Models: The recall involves Oceanic-brand Versa Pro 2A dive computers with serial numbers 12000 through 18176 which can be accessed and viewed on the computer’s display. The serial number and “r2A” are printed on the side of the unit. This recall does not include any other Oceanic brand dive computer, or Versa Pro 2A while operated in normal decompression dive computer mode. Sold at: Authorized Oceanic dealers sold Versa Pro 2A digital dive computers nationwide from March 2003 through April 2006 for between $430 and $640. NOTICE REGARDING D9 AND D6 DIVING INSTRUMENTS
June 9, 2006 Suunto Oy today announced an important safety notice regarding its D9 and D6 diving instruments. This global decision involves D9 and D6 instruments that have been delivered to distributors and retailers, as well as product that has been sold to consumers. Quality and safety are paramount concerns for Suunto Oy. Our testing program for the D9 and D6 product by Suunto employees and independent divers all over the world has been extensive. Despite this, Suunto Oy has identified a software bug in the D9 and D6 instruments. The software bug may cause the D9 and D6 to incorrectly track dive time on rare occasions, potentially causing a risk to the diver. The products affected
are: The software bug may activate when the D9 and D6 automatically recalibrate to keep the correct time, or the user manually updates the seconds in set mode. Suunto approximates that there is 0.6 % (a six-tenths of one percent) chance that the software bug may appear during a dive. When the dive time is incorrect, every fourth second is not registered in the dive time and surface interval time, i.e., dive time goes 25% slower than actual time and tissue loadings are recorded at 25% less than actual loadings. As a trained diver you understand that this could affect your decompression requirements and that the length of a dive is one of the key factors in determining those requirements.
Even though the D9 and D6 will function correctly approximately 99.4% of the time during a dive, Suunto wants to eliminate the very small possibility of recording the dive time incorrectly. Therefore, Suunto is requesting that all D9 and D6 shall be returned for a software update that eliminates this issue. To date, there have been no known injuries related to this issue. Action that YOU should take ˇ Suunto strongly recommends that you should not use the D9 or D6 for diving without backup instrumentation before the unit has been updated with the latest software. Please bring your D9 and/or D6 to your nearest local authorized Suunto retailer for the software updated as soon as possible. ˇ Please contact your local authorized Suunto retailer for contact information please visit our web site http://www.suunto.com/ You will then be advised on how to bring your D9 and/or D6 to the retailer for a software update. Once the retailer receives your D9 and /or D6 they will then forward your unit to the Suunto Service Center for the software update. Hazard: A manufacturing error could cause the main housing of some regulators to change shape over time, which could cause the cover and diaphragm to become dislodged. If this occurs, air flow will be interrupted and the regulators will no longer function, posing a drowning hazard to users. Incidents/Injuries: None reported. Oceanic Worldwide Announces Voluntary Recall of Versa Pro
revision 2A Dive Computers
May 3, 2006 – San Leandro, CA – In cooperation with the U.S. Consumer
Product Safety Commission (CPSC), Oceanic Worldwide of San Leandro,
California, is voluntarily recalling Oceanic Versa Pro revision 2A
Digital Dive Computers. While operating in the User Selected Digital
Gauge Mode, displayed Elapsed Dive Time can be in excess of actual
elapsed time.
Oceanic has received a report of two Versa Pro revision 2A units that
experienced the offset time displayed while operating in User Selected
Digital Gauge Mode. No injuries have been reported.
Oceanic Versa Pro 2A Dive Computers subject to the recall are only
the revision 2A units which have serial numbers 12000 through 18176
which is located on the side of the module and can be viewed if after
the front button is depressed to activate the unit, it is held depressed
until the Diagnostic Mode screen completes its countdown and the Serial
Number screen appears displaying the module’s SN and software firmware
revision (r2A). The unit will shut Off when released.
Authorized Oceanic dealers sold Versa Pro 2A modules, that were
manufactured between March 21, 2003, and February 7, 2005, in wrist and
console mount configuration during March 2003 through April 2006 for
between $429 to $639, depending on the configuration with other
instruments.
Consumers should immediately discontinue further use of Versa Pro 2A
DiveComputers for User Selected Digital Gauge Mode operation (as a Depth
Gauge/Timer) and take it to any Authorized Oceanic Dealer, or return it
directly to the factory for an Upgrade to Versa Pro revision 2B.

Description: The Aqua Scout Water Scooter is a hand-held, battery-powered product used to propel swimmers through the water. The product has a yellow or red plastic enclosure, a black plastic handle and propeller. There is no model numbers or brand name written on the scooter. Aftermarket Swivel for AGA/Interspiro Divator MKII Full Face Mask M&J Engineering Voluntary Recall of SW-3000 2nd Stage Swivel with Stamped date of 01/04 to 02/05 M&J Engineering voluntarily conducted the following product safety recall. Consumers should stop using the product immediately and contact M&J Engineering. Name of Product: Swivel SW-3000 Products Affected: All SW-3000 Swivels that are stamped with the manufacturing date of 01-04 thru 02-05 Manufacturer: M&J Engineering Hazard: The Swivel has the potential to become unscrewed while in use and at depth on the Interspiro Mask. This recall is for the AGA Nut and Washer only. It has come to our attention that there is a defect in the washer. The washer is too thick for the spec range and has the potential to lock against the nut while under pressure. While the insert is locked against the washer it has the potential to act as a ratcheting affect and will back the swivel nut off of the regulator. Incidents/Injuries: There have been a few incidents that have been recently brought to our attention that the swivel in the above described dates have unscrewed from the face mask while at depth. There have been no reports of injury. Description: The recall involves all SW-3000 2nd stage 360-degree swivels with the stamped manufacturing date of 01/04 thru 02/05. The Stamp is marked on the Swivel body underneath the Patent number. How to Identify the Problem: To identify which swivels are potential problems please conduct the following steps. Screw the swivel onto the mask 2nd stage regulator. Hand tighten the swivel until it is snug. The Swivel should be able to move in and out, pushing the ball of the swivel in towards 2nd stage regulator. It should move approximately 20-30 thousandths of an inch, (.020 to .030) or the approx thickness of 2-3 business cards. The Swivel should also rotate inside the body of the regulator smoothly with very little resistance. This is natural resistance from the o-ring inside the regulator body. If there is movement in and out and the swivel is free moving inside the body of the regulator proceed to the next step. If there is no in and out movement or the Swivel locks tight when screwed down, STOP use of the swivel IMMEDIATELY and refer to Remedy below. Next connect the system to an air tank. Apply standard pressure to the system as if you would normally do during your pre-dive check. Hold the hose and the mask. Move the mask around in a circular motion as to imitate the normal movement of your head while you are underwater. After you have done this at least five (5) times stop and check the swivel nut to see if it as backed of any. You will be able to thread the swivel down some. While the system is charged with air you will still be able to thread the swivel down if any movement has occurred. If the swivel has moved at all STOP use of the swivel IMMEDIATELY and refer to Remedy below. This is the swivel must have the washer replaced. Sold at: Diving Stores Throughout the USA. Manufactured in: USA Remedy: The washer will be replaced by manufacturer free of cost. In some instances it maybe necessary to replace the AGA Nut. If this is necessary the manufacturer will do so free of charge. Please send all SW-3000 AGA Swivels within date range stated above to M&J Engineering for replacement. Hazard: A slow leak from the shoulder exhaust caused by expansion of

internal cable housing could result in slow deflation. This could
impact
the diver's ability to control buoyancy.

Description:
Buoyancy Compensator devices provide buoyancy control for
scuba divers by allowing them to inflate or deflate the devices.
The recalled buoyancy compensators have model numbers J107, J113,
J115, and J119. The numbers appear on the top right-hand corner of the
unit (on
the shoulder). "CRESSI" is written on the buoyancy control devices.
Sold at:
Authorized dive shops nationwide and at various Web sites from
January 2003 through September 2004 for between $400 and $700.
Manufactured in: Genoa, Italy.
Remedy: Customers can return the product to a Cressi-sub-authorized
dive
shop to have the product retro-fitted with a free replacement
internal
cable housing. Buoyancy Compensator devices with a black retaining
ring
around the yellow inflate button on the inflation mechanism, and an
adjoining gray cover assembly (bypass case), which screws onto the
inflator, are subject to this recall and must be retro-fitted.

Incidents/Injuries: Head USA received one report of a computer malfunction. No injuries were reported. Description: The recall includes the Dacor Darwin Air dive computer with serial number 3265 through 3415, and the Dacor Darwin Console dive computer with serial numbers 8503 through 8592. The serial number will appear on the dive computer screen after removing and reinserting the batteries. HTM Sport S.p.A., Manufacturer of Dacor Dive Computers, has discovered that during production, a small number of Dacor Darwin Dive Computers were improperly calibrated, resulting in erroneous calculations displayed regarding "No Deco Time", "Deco Time", "Air Time", "Desat Time" and "No Fly" times. To locate the
serial number of your Darwin Air or Darwin Console:
1. Find the watertight battery compartment cover located on the top of Darwin Air or the left side on the Darwin console. 2. Unscrew the watertight cover by turning 90° counter clockwise. 3. Remove the batteries and then put them back in, after a few seconds the serial number of the instrument will appear on the screen. If you own one of the Darwin Computers with a serial number

Name of Product: Rotating Cross Bar SCUBA Manifold
Units: About 140 Importer: Ocean Management Systems, Inc., of
Middletown, NY.
Hazard: Undersized o-rings installed on the cross
bar can cause a loss of
breathing air from the SCUBA cylinder, presenting a risk of drowning.
Incidents/Injuries: There has been one report of
breathing air leaking from a cylinder. No injuries have been reported.
Description: The recall involves the 200 and 300 Bar
versions of the OMS Rotating Cross Bar Manifold. The manifold consists
of two valves which have "O.M.S." stamped on the side of each valve and
a crossbar that has three O-rings per side. Each part has a black
plastic knob.
Sold at: Dive stores nationwide from December 2003 through September 2004 for about $238. Manufactured in: Italy Remedy: Affected units will be replaced at no cost
to the consumer.

Affected are AERIS AW3 model Inflators that are fitted on AERIS Atmos XT BCs that were sold between May 1, 2004, and June 24, 2004. BCs sold prior to May 1, 2004, do not have these Inflators and are not affected. Testing and evaluation has shown that the Power Inflator Button may stick in the depressed position. Engineers have designed new replacement parts that will prevent the Button from sticking. AERIS asks that all owners of AERIS Atmos XT BCs that have AW3 Inflators return them to an Authorized AERIS Dealer for a retrofit at no charge. WARNING: DO NOT CONTINUE USE of the BC or attempt to disassemble any components of the Inflator. Take the BC with Inflator to your nearest Authorized Oceanic Dealer for inspection and service. Only Authorized Oceanic Dealers and their trained service technicians must perform this retrofit. | April 8, 2004 | Aqua Lung ABS Octopus Exhaust Grid Replacement http://www.aqualung.com/technical_library/ABSRetrofitInstorePoster.pdf Aqua Lung has determined that it is possible, under certain circumstances, for one or both of the exhaust valves on the ABS Octopus to become inverted. Inverted means that the edge of the exhaust valve can get lodged between two of the spokes of the seating surface. This could happen during the after-dive rinsing process if the valves are hit with a stream of high pressure water from a garden hose or it the valves hit the water just right during an entry. The result of having one or both valves inverted is the inability to completely seal out water. This could lead to an aspiration of water, which in turn could lead to panic, injury or death. The valves will not invert during the course of a dive. To protect the valve from inverting, Aqua Lung has redesigned the exhaust grid that covers the exhaust valves (see picture on website) Aqua Lung is requesting that all ABS Octopus regulators be brought
into your local authorized Aqua Lung Dealer. The dealer will
change out the exhaust grid at no charge. The replacement
procedure is quick and in most cases can be done while you wait.
The procedure is non-invasive and will not alter the performance or the
adjustments of the regulator in any way. Aqua Lung apologizes for any
